Primary care for all children in the family
Same-day or next-day sick visits

Membership Cost:
Birth to 26 years -- $125 per month*
One-time enrollment fee of $100 for first child and $50 each additional child. Max of $250
10% discount and waived enrollment fee if paying annually for membership
Max family monthly cost - $300
*Special "4th Trimester Package" - Parents may select to have the option of home visits with the doctor during the first 4 months for an additional onboarding fee -- $500 per newborn.
One-time sick visit or evaluation is available without membership - $245 single visit fee

What's Included in Your Child's Membership:
Flexible partnership model— You may choose Dr. DeForest as your pediatrician or continue with your current PCP and have Dr. DeForest support your child's complex medical needs.
Complex primary pediatric care for your special needs child
Direct, unlimited access to Dr. DeForest via clinic visits, telehealth, phone, and text messaging
Same-day or next-day visits
Extended comprehensive health visits to address dynamic healthcare needs
Care coordination and advocacy, including communication with specialists and/or school officials for optimal navigation
Nutrition and weight management including g-tube support
Dedicated care for dependent young adults up to age 26 who would benefit from comprehensive, supportive, family-centered primary care
Membership Cost:
Birth to 26 yrs -- $150 per month

A portion of the fee may be applied to a membership for continuing care if family decides to join the clinic for autism follow-up and ongoing management.
